Although today's 70 is yesterday's 60, older adults still face challenges that come with aging. Health issues, financial concerns and difficulties managing a home can bring stress to an older adult and the family. Our Creative Case Management Professional Geriatric Care Managers (GCM) listen to seniors and connect them with appropriate resources.

Senior care management includes:

• Advocating for the elderly and others with special needs.

• Looking for signs of physical, emotional, or financial exploitation and getting help.
• Counseling families, and working to sort things out.
• Finding in-home services or suitable independent/assisted living communities.

• Working as part of a care team where you are in charge.
• Improving quality of life for you and your loved one by providing support and expertise to help make sound decisions on health, social and living arrangements.
• Customizing a care plan and monitoring the care providers to make it work.
• Adjusting a care plan as needs of the situation change.
